血液透析联合血液灌流对肾性骨病的治疗作用研究

　　【摘要】 目的：研究与分析血液透析联合血液灌流对肾性骨病的治疗作用。方法：选取本院2016年1月-2017年6月收治的肾性骨病患者50例为研究对象，按照随机分数字表法将其为对照组和观察组，各25例。对照组采取血液透析治疗，观察组在对照组的基础上联合血液灌流治疗。观察对比两组治疗前后血清甲状旁腺激素（iPTH）、血钙、血磷等水平变化以及治疗效果。结果：治疗前，两组iPTH、血钙、血磷比较，差异均无统计学意义（P>0.05）；治疗后，观察组iPTH、血磷水平均低于对照组，血钙水平高于对照组，差异均有统计学意义（P<0.05）。观察组治疗总有效率为92.0%，高于对照组的72.0%，差异有统计学意义（P<0.05）。结论：血液透析联合血液灌流对肾性骨病的治疗作用非常显著，既能显著改善患者的临床症状，又能改善其血清甲状旁腺激素、血钙、血磷等水平，从而达到较好疗效，值得推广应用。

　　【Abstract】 Objective： To study and analyze the hemodialysis combined with hemoperfusion in treatment of renal osteodystrophy.Method：50 patients with renal osteodystrophy in our hospital from January 2016 to June 2017 were selected as the research objects，according to the random digital table method，the patients were divided into control group and observation group，25 cases in each group.The control group was given the blood dialysis treatment，on the basis of this，the observation group combined with hemoperfusion.The changes of serum parathyroid hormone（iPTH），serum calcium and serum phosphorus before and after treatment and effect of the two groups were observed and compared.Result：Before treatment，there were no significant differences in iPTH，calcium and phosphorus between the two groups（P>0.05）.After treatment，the levels of iPTH，serum phosphorus of the observation group were lower than those in the control group，the level of serum calcium in the observation group was significantly higher than that in the control group（P<0.05）.The total effective rate of treatment in the observation group was 92.0%，which was higher than 72.0% in the control group，the difference was statistically significant（P<0.05）.Conclusion：The effect of hemodialysis combined with hemoperfusion on the treatment of renal osteopathy is very significant，it can improve the clinical symptoms of the patients and the levels of serum parathyroid hormone，serum calcium， serum phosphorus and so on，so as to achieve a better curative effect，it is worthy of popularizing and applying.

　　在臨床上，肾性骨病又称肾性骨营养不良，是一种骨代谢紊乱疾病，属于慢性肾脏病的常见并发症。该病症主要临床症状为甲状旁腺代谢紊乱、血磷高、血钙低等。目前肾性骨病的发病机制尚未明确[1-3]。而肾病患者采取血液透析治疗的过程中，只能将小分子物质清除，不能将蛋白结合毒素以及中、大分子毒素等有效清除[4-6]。因此本文就血液透析联合血液灌流对肾性骨病的治疗作用进行了研究与分析，现将研究结果报道如下。
