归脾汤治疗气虚血瘀型慢性充血性心力衰竭临床观察

　　[摘要] 目的 探討归脾汤加减在气虚血瘀型慢性充血性心力衰竭（CHF）患者中的治疗效果。方法 选择2016年1月～2017年6月我院中医内科收治的136例气虚血瘀型CHF患者随机分为观察组（n=68）与对照组（n=68），对照组给予常规抗心衰治疗，观察组在对照组基础上加用归脾汤治疗，对比两组患者临床效果、心脏功能和静脉血Pro-BNP及CRP变化。结果 观察组有效率为98.5%（67/68），对照组有效率为82.4%（56/68），观察组有效率高于对照组（P<0.05）；两组患者治疗前LVEF、LVEDD以及LVESD比较均无统计学差异（P>0.05），治疗后两组LVEF增加（P<0.05），LVEDD以及LVESD降低（P<0.05），治疗后观察组LVEF高于对照组（P<0.05），LVEDD以及LVESD低于对照组（P<0.05），两组患者治疗前Pro-BNP及CRP比较无显著差异（P>0.05），治疗后两组Pro-BNP及CRP均较治疗前降低（P<0.05），观察组治疗后Pro-BNP及CRP低于对照组（P<0.05）。 结论 归脾汤与传统抗心衰药物联合用于气虚血瘀型CHF的治疗，能够有效改善患者临床症状体征，提高患者心功能，具有较好的临床效果。

　　[Abstract] Objective To investigate the therapeutic effect of Guipi decoction on patients with chronic congestive heart failure（CHF） of vital energy deficiency and blood stasis type. Methods 136 patients with vital energy deficiency and blood-stasis type CHF who were admitted to our hospital from January 2016 to June 2017 were randomly divided into observation group （n=68） and control group （n=68）， and control group took anti-heart failure treatment， the observation group took treatment based on the control group plus Guipi decoction. Clinical efficacy， cardiac function and venous blood Pro-BNP and CRP changes in both groups were compared. Results The effective rate was 98.5%（67/68） in observation group and 82.4% （56/68） in control group， and the effective rate in observation group was higher than that in control group （P<0.05）. The LVEF increased in the two groups after the treatment （P<0.05） while LVEDD and LVESD decreased （P<0.05）， LVEF in the observation group was higher than that in the control group after treatment （P<0.05）， LVEDD and LVESD were lower than those of the control group（P<0.05）. There was no significant difference in Pro-BNP and CRP between the two groups before treatment（P>0.05）. The Pro-BNP and CRP decreased in the two groups after the treatment （P<0.05）， Pro-BNP and CRP in the observation group were lower than those in the control group （P<0.05）. Conclusion Guipi decoction combined with traditional anti-heart failure drugs for the treatment of CHF with vital energy deficiency and blood stasis can effectively improve patients" clinical signs and symptoms and improve cardiac function， and has good clinical effect.
