ADAM—15在WHOⅠ级脑膜瘤中的表达及其与侵袭性的相关研究

  【摘要】 目的:探究ADAM-15在WHOⅠ级脑膜瘤中的表达情况,进一步分析其在WHOⅠ级脑膜瘤侵袭性生长中的意义。方法:收集63例WHOⅠ级脑膜瘤标本(33例有侵袭性,30例无侵袭性)为试验组,21例正常脑膜组织为对照组。通过免疫组织化SP法检测去整合素-金属蛋白酶15(A Disintegrin and Metalloproteinase 15,ADAM-15)在上述不同脑膜组织中的表达情况。结果:试验组ADAM-15的阳性表达率高于对照组,差异有统计学意义( 字2=17.296,P<0.001)。侵袭性组ADAM-15的阳性表达率高于非侵袭性组,差异有统计学意义( 字2=8.376,P=0.004)。侵袭性组ADAM-15的阳性表达率高于对照组,差异有统计学意义( 字2=24.661,P<0.001)。非侵袭性组ADAM-15的阳性表达率高于对照组,差异有统计学意义( 字2=6.801,P=0.009)。结论:ADAM-15在WHOⅠ级脑膜瘤侵袭性生长中起一定作用。

  脑膜瘤是颅内常见的良性肿瘤,约占原发于颅内肿瘤的30%[1]。脑膜瘤通常为良性肿瘤,但其具有广泛的临床特征和组织学分类,根据WHO中枢神经系统肿瘤的分类[2],将脑膜瘤分为三个级别共15个亚型,良性脑膜瘤(WHOⅠ级)复发和侵袭性生长的风险低,非典型脑膜瘤(WHOⅡ级)更可能复发,而间变性(恶性)脑膜瘤(WHOⅢ级)复发的可能性最大和/或具有更强的侵袭性。良性脑膜瘤在组织病理学方面无明显差别,但有一部分良性脑膜瘤细胞表现出向周围组织浸润生长的侵袭性生物行为。脑膜瘤目前的治療手段仍以手术切除为主,虽然良性脑膜瘤理论上手术全切可以治愈,但是部分良性脑膜瘤因其对周围组织的浸润而无法行全切手术,部分脑膜瘤即使手术全切,术后也有一定的复发率。研究良性脑膜瘤侵袭性生长的相关因素,寻找有效的诊断及预测其侵袭性的生物学指标非常重要。去整合素-金属蛋白酶15(A Disintegrin and Metalloproteinase 15,ADAM-15)是一种Zn2+依赖的Ⅰ型多结构域跨膜内肽酶,属于ADAM家族20多个成员中重要的一员[3-4],活化的ADAM-15参与细胞生长以及细胞外基质(ECM)中Ⅳ型胶原和明胶的蛋白水解[5-6]。ADAM-15参与了人类许多疾病的发展,尤其是在肿瘤的侵袭性生长中起非常重要的作用。本试验通过检测ADAM-15在WHOⅠ级脑膜瘤中的表达情况,研讨ADAM-15在WHOⅠ级脑膜瘤侵袭性过程中的作用,为临床提供有价值的WHOⅠ级脑膜瘤侵袭性生长的预测因子,现将研究结果报道如下。
