实验室指标对多发性骨髓瘤的诊断价值分析

　　摘   要：目的  分析多发性骨髓瘤实验室结果对多发性骨髓瘤的诊断价值。方法  以2015年1月～2017年5月我院收治和确诊的28例的多发性骨髓瘤患者作为研究对象，回顾分析其室验室指标的特性，包括HBG、UA、Scr、TP、GLO、A/G、ESR、B-J、β2-MG、尿蛋白定性、IgG、IgA、IgM。结果  28例MM患者中，有26例（92.86%）HBG<90 g/L，25例（89.28%）UA、Scr升高，22例（78.57%）TP升高、22例（78.57%）GLO升高，25例（89.28%）A/G倒置，28例（100.00%）MM患者ESR明显升高，14例（50.00%）B-J阳性，26例（92.86%）尿蛋白定性阳性，26例（92.86%）β2-MG升高，19例（67.86%）IgG明显升高，6例（21.43%）IgA明显升高。结论  HBG、UA、Scr、TP、GLO、A/G、ESR、IgG、IgA等实验室结果对临床诊断MM有一定的灵敏度和特异性，临床检验人员应引起重视，加强与临床沟通，做到早诊断、早治疗，防止误诊、漏诊。

　　Diagnostic Value of Laboratory Markers for Multiple Myeloma

　　Abstract：Objective  To analyze the diagnostic value of multiple myeloma laboratory results for multiple myeloma.Methods  In January 2015～2017 year in May in our hospital and diagnosed 28 cases of multiple myeloma patients as the research object，analyzed its characteristics of room laboratory indexes，including HBG，UA，Scr，TP，GLO，A/G，ESR，B-J，2-MG，β2-MG，qualitative urine protein， IgG，IgA，IgM.Results  Among 28 cases of MM，26 cases（92.86%）HBG<90g/L，25 cases（89.28%）increased UA and Scr，22 cases（78.57%） increased TP，22 cases（78.57%）GLO increased，25 cases（89.28%）AG reversed.The ESR of 28 patients with MM（100.00%）was significantly higher，14 patients（50.00%）were positive for B-J，26 patients（92.86%）were positive for urinary protein，26 patients（92.86%）were elevated for β2-MG，and 19 patients（67.86%）IgG was significantly elevated，and IgA in 6 patients（21.43%）was significantly higher.Conclusion  The laboratory results of HBG，UA，Scr，TP，GLO，A/G，ESR，IgG and IgA have certain sensitivity and specificity for clinical diagnosis of MM.The clinical laboratory staff should pay attention to it，strengthen communication with clinic，make early diagnosis and early treatment，so as to prevent misdiagnosis and missed diagnosis.

　　多發性骨髓瘤（multiple my eloma，MM）是发生在人体骨髓的多灶性浆细胞异常增生的恶性肿瘤，是恶性浆细胞疾病中最为常见的一种。MM起病隐匿，临床表现复杂多样，且无明显特异性，易导致误诊、漏诊。主要表现在血液、骨胳、肾脏等方面的损伤，易诊断为血液、骨科、肾脏疾病，造成误诊[1-2]。为了进一步促进早发现、早诊断、早治疗MM，现以2015年1月～2017年5月我院收治和确诊的28例病例作为研究对象，分析、探讨实验室系列指标对此类疾病的临床诊断价值。
